Background and Objectives: The 12th grade health and environment textbook is the last educational opportunity before students enter adult society. Given the increasing changes in adolescent health, including the expansion of the digital space, climate crises, and psychosocial complexities, this study examines whether the content of the book is consistent with the life issues of adolescents. The main objective was to simultaneously critically analyze the quantitative and qualitative structure of the book within the framework of critical health literacy, social determinants of health, and critical localization. Methods: This study was conducted with a mixed approach. Quantitative analysis included counting and coding all textual, visual, and educational activities in 14 lessons of the book (a total of 172 pages). At the same time, qualitative analysis was applied to the content of the lessons using the thematic analysis method. To ensure the reliability of the qualitative analysis, the degree of agreement between the two coders was calculated using the kappa coefficient. The obtained value (0.74) indicated good agreement. Findings: The book has adequate coverage of traditional health areas and Islamic-Iranian teachings. However, important new topics such as digital health, mental health in cyberspace, climate change, gender equality, critical health literacy, and pandemic preparedness are not considered. The dominant discourse of the book is individualistic and behavior-oriented and rarely addresses structural and social factors affecting health. Also, references are mostly focused on domestic and religious sources, and multicultural dialogue with global health knowledge is limited. Conclusion: Despite establishing cultural-religious identity, the present book faces structural shortcomings in reflecting current issues of adolescent health. A content review with a forward-looking, critical, and inclusive approach is necessary so that textbooks can cultivate informed, responsible, and participatory citizens.
